The Neuro Frequency Investigation Initiative (NFII), a project of Cognitive Freedom Coalition, collects participant accounts and connects research questions with investigative institutions and universities.
This questionnaire records your experiences and your own understanding of them.
It does not determine their cause.
The current intake focuses on people reporting experiences they associate with frequency-based effects, RNM, V2K, or directed energy; reports limited to being followed or watched fall outside this intake.
Study participation is in person only.
Locations and arrangements will be provided with any study invitation.
Completing this form joins a participant registry, not a particular study.
No study placement, payment, finding, or direct benefit is promised.
You may stop before submitting, skip optional questions, decline later invitations, or request withdrawal without penalty.
The safety questions help the team plan in-person evaluations; the team reviews these responses before arranging a visit.

NFII stores your contact details, general location, responses, and dated consent in its private database.
Authorized NFII reviewers may access your record.
Information is shared only for the purposes you authorize, not posted publicly.
Sharing may involve institutions in other countries; confidentiality cannot be guaranteed once information is shared.
Records are retained while needed for the initiative unless you request removal.
A withdrawal request stops future contact and sharing after your identity is verified; previously shared information or completed analyses may not be recoverable.
